Systemic: Dizziness / Lightheadness-Medium, Systemic: Flushed / Sweating-Medium, Systemic: Hyperventilation-Medium, Systemic: Shakiness-Medium, Systemic: Tingling (specify: facial area, extemities)-Medium, Systemic: Visual Changes/Disturbances-Medium, Additional Details: Pt reported having difficulty breathing, auditory and visual changes, shakiness, and dizziness. Patient states that she has a history of panic attacks and anxiety. Pt said she had difficulty getting vaccines in the past due to anxiety and dizziness but hasn't had issues in the recent past. Pt was given water and lied down on the floor. 911 was called and BP, medical hx was assessed. Pt reported feeling better and opted to not get taken to the hospital by EMS.
