Symptom narrative

Site: Itching at Injection Site-Medium, Site: Pain at Injection Site-Medium, Site: Swelling at Injection Site-Medium, Systemic: Allergic: Itch Generalized-Medium, Additional Details: Patient came back to the pharmacy 3 times since vaccination. Patient was advised to apply cold pack, and to take benadryl capsules during the initial report. Patient was advised to contact PCP or go to the walk in clinic for follow up. During todays meeting , i did not observe any visual redniss , fever or other illness however patient says it is red , itches and is hard inside. Patient was asked again to follow up with pcp. Dr was also callled (spoke to MA) and informed about it.
