Symptom narrative

After receiving HPV vaccine, pt started to feel dizzy/lightheaded, sweaty and was very pale. BP taken, legs were elevated. After patient started to feel better, pt was moved to a exam bed, and rested there until feeling better. BP was taken 4 times.

Current illness

Did have UTI 9/28/25, was treated for this

Depression, ADHD

Medications were not taken at time of vaccination,. Pt does take daily Adderall, Bupropion, Lexapro, Guanfacine, Estradiol, Testosterone, unclear as to when these medications were taken(none taken at office visit where vaccine was administe
