Symptom narrative

Vaccines given 10-9-25. Mom called the office 10-20-25 to report that on 10-10-25 patient woke up with swollen lips. She also complained of sore throat. Did not seek medical attention at that time. The swelling was gone by 10-11-25. Mom took pictures of patient's lips on the day it occurred.
