The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 06-Oct-2025, the patient was inadvertently vaccinated with the first dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (valid lot #Y006783, expiration date: 24-Sep-2025) for prophylaxis (expired product administered). The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported). No temperature excursion had occurred.
