Patient received Fluad from the office yesterday and then when returned home daughter called to let us know he had gotten his Flu shot at (withheld location) on 9/12/25. Patient did not recall getting the flu shot on 9/12/25 when patient was in and office. Flu shot was not reported by (withheld) on Database.
