Symptom narrative

Pharmacy was having trouble accessing database, patient came in to receive her 2nd vaccine. Pharmacy gave it to her, unfortunately realized they had given patient a 3rd dose of the vaccine when database did come back online. There are no adverse reactions reported by the patient.
