Symptom narrative

pt came by the pharmacy to pick up RXs and reported having a rash since getting the vaccine on her upper R arm by the elbow that is about 4 inches in diameter. It was not itchy, improving, so pt didn't contact the prescriber. I could not see the rash when she showed me. Pt said it was there till today but a lot better. PT didn't take any medication for it.
