Symptom narrative

Beginning the day after the vaccine (10/15/2025), patient experienced pain, swelling, and a welt at the injection site. The area of swelling and redness expanded down to the patient's elbow. Today (10/22/2025), the swelling is going down, but the patient continues to feel malaise and fatigue that she associates with the adverse reaction.

Medical history

Hypothyroidism, Primary Hypertension

Other medications

Vitamin D2 1.25mg by mouth once weekly, Synthroid 50mcg by mouth once daily, Synthroid 25mcg by mouth once on Monday, Wednesday, and Friday, Propranolol 20mg by mouth once daily

Allergies

Penicillins (Amoxicillin): Reaction was hives, Codeine: unknown reaction, Erythromycin (unknown reaction), many other antibiotics (unknown which ones, unknown reaction)

Prior vaccinations

Pt always experiences extreme fatigue and flu-like symptoms after any vaccinations
