Symptom narrative

On 10/22/2025, at facility, RN administered the wrong vaccine. He said ?Polio vaccine? and injected me subcutaneously in the back of my upper arm using a white box with green lettering starting with ?P.? I was supposed to receive the HPV (Gardasil 9) vaccine. The error was discovered shortly after when the front desk and other staff realized he was not authorized to bring me back. Another MA later administered the correct HPV (Gardasil 9, IM) vaccine the same day. No documentation of the incorrect vaccine appears in MyChart as of now.
