Symptom narrative

The school nurse called. States the mom of student called upset that she was given a flu shot at the school flu clinic today. Mom said she is pretty sure she was given the flu shot when she came to the HU on 10/6/25 for routine vaccines. The school nurse said that she had called mom a few weeks ago to let her know she was behind on vaccinations. They discussed getting the flu shot at her appointment at the health department that day too, but mom wasn't sure if she wanted to do it then or not. She said she might wait and just have her get it at school. The school nurse said she had brought in a copy of her shot record after her appointment that had the vaccines, she was given 10/6/25. She did not see a flu shot given that day so assumed she decided to wait, she did not realize that those don't get entered into web iz right away. So she left her flu form in the stack and sent her to get her shot today. The mom was educated that if she developed any type of abnormal allergic reactions or just anything abnormal to go to the ER and let them know what had happened. School nurse reported that patient has been back at school and mom has not reported any problems.
