Symptom narrative

No adverse event, but an administration error. The patient had signed up to receive the Prevnar 20 vaccine in addition to his flu and covid, however he had already received the Prevnar 20 vaccine earlier this year. We reversed the prescription but it wasn't properly documented on his sign up sheet to not give that vaccine, and therefore it was still given.
