Symptom narrative

Incorrect vaccine administered to patient. Patient was due for the FLU and PCV vaccines. I accidentally administered the HEP B ADULT vaccine instead of the PCV vaccine. I realized I administered the incorrect vaccine when entering it in to EPIC. Patient was notified immediately and had no problem. Chose to schedule PCV administration a month out. My supervisor was notified by me as well as the patients PCP.
