Symptom narrative

Patient presented with red, splotchy, and warm circle, roughly 5" in diameter, on left arm around injection site. Patient stated that t did not appear until 3 days after injection. Patient stated that the person administering the vaccine did not use an alcohol swab prior to administration. She was seen at the local clinic 3 days after injection and prescribed cephalexin 500mg BID for 10 days.
