Symptom narrative

A 15 year and 11-month-old patient was administered the Meningococcal (MenACWY) and Tdap vaccines. Upon reviewing the record afterward, I realized that I mixed the 2 kids up. Both received the HPV vaccine and Flu mist, but the 11-year-old should have had the HPV, meningitis, TDAP and flu mist. The 15.11 month should have received the HPV vaccine and flu mist only. The meningococcal booster was given before the patient s 16th birthday (recommended age for this dose). The Tdap vaccine was administered approximately five years after the last dose, which is earlier than necessary. >>>There were no adverse reactions or safety concerns, and both vaccines are non-live and safe when given early. However, I understand the MenACWY dose will need to be repeated after the 16th birthday to meet immunization requirements. I wanted to report this promptly for proper documentation and follow-up with the patient s record. Thank you,
