On 10/21/25 at approximately 10:05 AM, I administered the incorrect vaccine in Room 8 to the patient. I was working with the provider, who had ordered the Tdap vaccine. When I retrieved the vaccine from the AcuVax. I then consulted with the provider to verify the vaccine, and Dr. confirmed it was correct. Despite this, I had a gut feeling the vaccine might still be incorrect, so I asked another MA to verify it. She also confirmed the vaccine. I proceeded to administer the vaccine. However, when I attempted to enter the vaccine information into the system, I noticed that the lot number I was entering did not match the Tdap vaccine. This is when I realized that I had administered the DTaP vaccine instead of the Tdap. I immediately informed the provider about the incident. Dr. stated that it was not a major concern, noting that since the DTaP is a smaller dose intended for infants, the patient should be fine. She did not believe any medical intervention was necessary. Next, I informed my supervisor about the situation. She instructed me to notify our Compliance Manager about the incident. At 11:09 AM, I messaged our Compliance Manager about the incident. However, I did not receive a response. Below is the message I sent her via chat: ?Good morning, I accidentally administered the incorrect vaccine. Instead of the Tdap, I gave her the DTaP. What would be the next step??
