No additional AE reported; late delivery for PROQUAD; missed their dose; This spontaneous report was received from a medical assistant and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On an unknown date, the patient came in to receive measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) injection (dose, route of administration, anatomical location, lot # and expiration date were not reported) for prophylaxis, reconstituted with sterile diluent (lot# and expiry date were not reported), however, due to late delivery, the patient missed their dose (Product supply issue, Product dose omission issue). No additional adverse event was reported (No adverse event). Lot# is being requested and will be submitted if received.
