Symptom narrative

Wrong vaccination type was ordered (ordered 12y+ vaccine Moderna when should have ordered Pfizer 6m-11y vaccine), 3 year old patient received vaccination before error was determined. Patient had no immediate signs or symptoms of adverse reaction and was monitored for 15 minutes. Discussed error with family and reviewed possible adverse effects of this. Reviewed when to seek care in Emergency Department versus when to call our office.
