Symptom narrative

Vaccine (Hepatitis A) was administered at the wrong age. Patient was scheduled to receive Hepatitis B vaccination. No adverse reaction at the time of the injection. Mother stated recently this year to the staff that patient has been diagnosed with, "medical problems and Autism." No documentation within chart due to patient being transferred out of office after the incident.
