Symptom narrative

After vaccine, patient was getting out of chair and rph helped assist. Patients legs could not support and patient fell to the ground. She did not hit her head but was unable to get back up. RPh called 911. while waiting on help, patient was short of breath, complaining of knee pain, and felt lightheaded/dizzy. Pt daughter said she has fallen before and has vertigo. patient was then transported to hospital via ambulance.
