Symptom narrative

Patient came in October 21,2025 and mentioned he has a pain that started at the injection site which later radiated to the whole shoulder and to the back of the shoulder. No tingling, but here and there he feels involuntary muscle twitching around the shoulder. He has been taking ibuprofen 800 mg twice daily for the last week, but the pain is not going away. I recommended him to get seen by his doctor and patient confirmed he will do that.

Diabetes, Hypothyroidism, Hypertension

Amlodipine 10mg, Carvedilol 12.5 mg, Gemfibrozil 600 mg, Jardiance 10 mg, levoxyl 112mcg, Losartan/HCTZ 100/25mg, Metformin ER 500 mg
