Symptom narrative

Moderna 8/17/2021 , Moderna 7/13/2021, patient with spasticity, ataxia symptoms and exam consistent with seronegative Stiff person syndrome. Timeline of symptoms not clear as per records but patients reports slow and progressive symptoms after Vaccine COVID were done. Never filed report as was not aware of this.
