Symptom narrative

Right after giving the vaccine, patient arm bleed a little bit and and swell up. Band-Aid was removed and bump was cleaned with a cotton ball. the bleeding stopped and the arm had a bump at injection site. Color of the bump was normal skin tone. Patient instructed to monitor and place ice pack for swelling. also can use Tylenol for some pain relief.
