Symptom narrative

Patient was standing by the door of the vaccine room while his dad was getting a vaccine, and proceeded to fall. He claims he never lost conciousness, eyes were open the whole time. He fell against the door and hit shoulder, and sounded like he hit his head but said it didn't hurt and refused ice. Gave him a glass of water and asked them to sit him nearby for 30 to 60 minutes for observation. Said nothing like this ever happened before, and he felt okay. His sister was also in the room, but did not see him fall.
