Symptom narrative

Patient requested RSV vaccine as she wasn't sure if she had ever gotten it. RPh checked immunization registry and pt profile. No hx of RSV vaccine on either of those that pharmacist could see as patient profiles only go back by 2 years at the local level. Corporate's Clinical services notified RPh on 10/16/25 that vaccine had already been given in August of 2023. Administration in August of 2023 never reported to immunization registry. RPh looked for consent form from August and once found reported info.
