Symptom narrative

atient was given a 4th dose of Vaxelis (only 3 are required), and the minimal interval between Dtap 3 and Dtap 4 were not met. Clinic change of vaccine to vaxelis and this was ordered by provider. Correct order would be for DTAP and HIB separately. Received unnecessary IPV and Hep B components which were not required. Interval also not sufficient and vaccine is invalid.
