Symptom narrative

4-year-old client vomited after vaccine, no other symptoms. Nurse advised to go to ED / urgent care if other symptoms arise. Nurse monitored for 15 minutes. After vomiting parent reported that prior to vaccine administration, child had a "tummy ache" - this was not reported by parent when nurse asked if child was feeling well prior to getting the vaccine.
