Symptom narrative

Site: Redness at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Additional Details: Patient felt dizzy and fainted at the front store after receiving her flu vaccine. Ambulance was called, she got checked out, all vital signs were normal. She was given some water, she sat on a chair, rested for a little while and got sent home.
