Site: Redness at Injection Site-Mild, Additional Details: pt came in to get flu vaccine but received both covid vaccine (pfizer Comirnaty) and flu vaccine on 10/19/2025. Tech that gave her vaccine informed me she gave her covid vaccine but she requested only flu vaccine, after checking system she got covid spikevax on 09/10/2025. spoke to pt same day , 10/19/2025, she confirmed she is doing ok but will notify her dr or go to ER if she has any side effect symptomds
