Symptom narrative

Site: Itching at Injection Site-Mild, Site: Redness at Injection Site-Mild, Site: Swelling at Injection Site-Mild, Systemic: Allergic: Rash (specify: facial area, extremeties)-Mild, Additional Details: Patient came in monday 10/20/25 and showed site of inject with an area of red raised rash at injection site. Patient complained of only slight itching but redness has not improved or gotten worse since day of injection. Patient was concerned it was not given in the correct location. It was not given right in the center, just off a little to the left, but still given within the muscle. Patient was advised to seek medical attention if it spreads, gets more red or hot.
