Site: Pain at Injection Site-Mild, Systemic: No reaction noted after 45 minutes of observation. No reaction at injection site, discharged bandaid intact-Mild, Additional Details: Nurse gave high potency Fluzone, intead of "regular" Fluzone vaccine. Vaccine dose was not verified with scan by vaccine nurse(me) Sign on issue with scanner.
