Symptom narrative

Systemic: Chills-Mild, Systemic: Confusion-Severe, Systemic: Dizziness / Lightheadness-Severe, Systemic: Exhaustion / Lethargy-Severe, Systemic: Fainting / Unresponsive-Severe, Systemic: Hyperventilation-Severe, Systemic: Hypotension-Severe, Systemic: Shakiness-Severe, Systemic: Visual Changes/Disturbances-Severe, Additional Details: Pt reported to Mom that she could not see or hear for a couple minutes after receiving vaccination. Pt passed out and was in and out of consciousness, cold/clammy. hyperventilating. Called EMS. By the time pt left with EMS, could see and hear properly but was feeling faint and lethargic. Pt family reported pt was okay later in the day and ER had determined she had syncope/hypotension causing the passing out. Pt mom reported that pt had received flu shots and other vaccines with no issues.
