Systemic: Chest Tightness / Heaviness / Pain-Medium, Additional Details: Patient Assisted living reported that a patient, patient reported chest pain later on that day that did resolve. However, on 10/18/25 reported chest pain again and the assisted living did transport patient to hospital and they did find that patient had an ejection fraction <25% and was to be getting a heart catheter soon. That was all the information that they were able to provide for the patient.
