Symptoms (1)

Erythema multiforme

Symptom narrative

Acute onset of targetoid lesions c/w erythema multiforme following recent administration of Quadracel (DTaP-IPV) vaccine. No evidence of active infection or alternative drug exposure. No significant mucosal involvement or systemic symptoms at this time. EM is a rare, immune-mediated hypersensitivity reaction in children, most commonly triggered by infections, but vaccines are a recognized cause, particularly in the pediatric population. Discussed course is self limited with resolution expected in 7-10 days. Differential diagnosis includes urticaria and Stevens-Johnson syndrome; however, the fixed nature of lesions and absence of extensive mucosal involvement support EM. Mother to monitor for progression or systemic symptoms including mucosal involvement dehydration, etc. Conservative care with tylenol/motrin for pain, and antihistamines for any pruritis. Red flags and return precuations discussed.
