10-22-25 @ 15:05 2nd dose of highdose influenza vaccine administered instead of covid 19 vaccine in error. Monitored x 40 min, no S/E reported by patient. 10/27/25 Spouse reports no S/E noted by patient since 10/22/25 flu vaccination, agrees to have pt. update nurse and PCP if any no S/S developed
