Patient received two doses of the Flu HD. 8/29/2025 and 10/7/025.

Medical history

(HFpEF) heart failure with preserved ejection fraction, Join Pain, HTN, Family Hx of CAD, GERD, Hypothyroid, Nocturia. B/L Carotid artery Stenosis, Pre DM, Elevated AST, Elevated PSA, Severe OSA, R knee pain, Dry Seile Macular degeneration, balance disorder, stress, HSV-1/HSV-2, R shoulder tendonitis, Pulonary nodules, Eczema, Hearing reduced, sciatic leg pain, and impacted cerumen of both ears.

amLODIPine Besylate 10 mg Tab 1 PO QD Esomeprazole Magnesium 40 mg Cap delayed rel 1 capsule orally daily 30min before eating hydroCHLOROthiazide 12.5 mg Tab 1 po qd Ketoconazole 2 % Crm 1 application topically to affected area daily x
