So at the time of the appointment pt was here for a 1st regular wellcheck visit. She was worked up by 2 Nurses one did questions and the other did vitals. The Nurse who did the questions was the one who added the note needing a certain vaccine (MMR) and left the wrong one in (Prevnar 20). After the Dr. went in to evaluate the pt and she signed off the order that was inputted by the first nurse. i was the floating nurse that had to grab vaccine for the pt. i didn't double check if they already received that vaccine. So it was 3 different people and didn't catch it.
