Symptom narrative

no additional AE; patient was inadvertently administered PROQUAD; this dose was an extra dose; This spontaneous report was received from a medical assistant and refers to a 5-year-old patient of unknown gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 27-Oct-2025, the patient received an extra dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), Solution for injection (lot #Z011831, expiration date: 12-Jan-2027) 0.5 mL, administered by Intramuscular route in Right Arm (extra dose administered). It was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), Solution for injection administered by Intramuscular route (expiration date, and lot # were not reported). No additional adverse event was reported.
