On October 28, 2025, I was notified by the Medical Lead that I had administered the HPV vaccination to Youth on October 17, 2025, despite the vaccine already having been administered by another nurse on September 28, 2025. When the vaccine was administered on October 17, it was listed in the Orders Manager as a task needing completion. Prior to administering, I verified the Youth?s vaccination record through database, which at that time indicated that the Youth was not current on the HPV vaccination and required it to be given. Earlier in the month, the EHR system had crashed and the facility was operating under downtime protocol. During that time, the HPV vaccine administered on September 28 was never properly entered into system, as the system did not come back online until October 15. After system was restored, certain system glitches persisted, including issues with the interface used for system to communicate with database. Because interface was not functioning properly, the Youth?s record did not reflect the September 28 vaccination. As a result, on October 17, system and database both appeared to indicate that the HPV vaccine was due, and the vaccine was administered in good faith. After review and consultation with the Medical Providers, it was determined that Youth is not at risk of any medical complications or adverse effects related to receiving an additional dose. The Youth has not c/o any adverse affects from the vaccine administration.
