Patient scheduled appt for Hi Dose Flu Shot. Presents today for appt for shot. VIS reviewed with him. Signs consent. Immunization given. After patient leaves clinic, today's dose was entered into vaccine record. It was then discovered that per vaccine record, patient had received Hi Dose Flu shot 9/03/2025 by medical clinic. Client was contacted by phone and notified that he had previous shot and had received same dose again today. He agreed to contact his personal care provider and notify them and follow in advice they give to him. He was also told he could call this office and report any adverse reactions.
