After administering MMR, HEP B, and TDAP in clinic. Patient starts mumbling and leans back in chair. Patient had a brief syncopal episode after vaccine administration. Patient was nervous prior to receiving vaccines. RN supported patient during episode. Patient was unresponsive for 15-20 seconds. RN called for assistance and another RN came to assist. Client regained consciousness and was fully oriented and responsive. RN gave patient 2 bottles of water, crackers, and an icepack. Vital signs taken. BP at 9:15am was 110/68, HR was 80, and RR was 18. RN advised patient to accompany her and drive her and patient stated she felt fine and it wasn't possible for anybody to come drive her. Patient was stable on feet. Patient walking and talking like normal after event. Patient declined transportation.
