Symptom narrative

Patient was being seen for his 4 month wellness exam and vaccines. When it was time to to receive vaccines medical assistant got confused and got the MCV instead of PCV vaccine by mistake. Medical assistant did no realize mistake until it was time to add into patients chart. Patient was reached out and was told by mother, patient had fever and soreness to the injection site. No other complaints and patient is doing better.
