Symptom narrative

I was giving a flu shot to a husband and wife. I put the supplies for both shots in the basket to carry out. I gave the shot to the wife, capped the syringe and place it back in basket instead of placing directly into the sharps container. I then moved over to the husband and grabbed the syringe I used for the wife and inserted into the husbands arm then realized that the dose had already been used. I then grabbed the unused syringe and administered the dose.
