Symptom narrative

My daughter was seen for a routine visit at her pediatricians office. My wife and i had repeatedly expressed a desire to vaccinate according to a delayed schedule. This was reviewed at each prior visit and today included. On this visit, she was to get the oral rotovirus vaccine and TDaP injection. This was reviewed at length with the administering nurse prior to injection. When we returned home and reviewed her chart, we found that she was erroneously injected with the 4-1 DTaP polio hep B Hib injection against our wishes. The office has provided no explanation for this error.
