PATIENT WAS GIVEN COMBO VACCINE INSTEAD OF SEPERATE VACCINES FOR MMR AND VARICELLA. WAS WRITTEN DOWN IN A CONFUSING WAY AND WAS MISINTERPRETED. PATIENT IS AWARE AND IS NOT CONCERNED. PHARMACIST LOOKED INTO IT AND PATIENT DOES NOT NEED TO BE REVACCINATED.
