Symptom narrative

Patient reports that she was experiencing nausea, dizziness, and anxiety. She returned to clinic thirty minutes after vaccine was given. I sat with patient for an additional 30 minutes. The patient stated that she was not improving but not getting worse. She was alert and oriented. Her pulse was 71. I recommended that she be evaluated at local emergency room.
