Symptom narrative

Child reported nausea and experienced syncopal episode approximately 7 minutes after receiving immunization. No loss of consciousness. Child assisted to the floor to lay down while being monitored by clinic staff for approximately 3-4 minutes. Child requested to sit up, and was assisted into a chair by clinic staff. Child requested water which was provided. Child was escorted to school nurses' office by clinic staff, and child stated that he had not had breakfast and was feeling very anxious up to and during vaccination. Child was monitored by school nurse and clinic staff for approximately 30 minutes before returning to class.
