Symptom narrative

Patient called our pharmacy 10/28 stating that the patient had a severe adverse reaction to his flu vaccine he received from us on 10/11. He stated he had an infection from the site of the injection up to his neck then spreading to the jaw and 6 teeth. Per patient according to his autoimmune doctor he believes the vaccine was contaminated. The patient reported autoimmune doctor reported to VAERS on his behalf at the ER. He stated he has had flu shots in the past and with no issues.
