Symptom narrative

Patient stated she was vaccinated on 10/28/2025. Began with itchy, redness, warm to touch raised bumps, one day following. It worsened on second day and came to Center. Client stated unable to know which vaccine caused reaction. Pt. explained no further knowledge of what happened during encounter. During today's encounter (12/01/2025) pt declined Varicella vaccine due to possible worsening reaction. Other vaccines given without concern (Polio, Hepatitis B, MMR) Informed patient to return to clinic if any adverse signs or symptoms arise such as last time. Client verbalized understanding.
