Symptom narrative

Patient was given the intramuscular injection Fluzone High dose in the left deltoid using a 5/8 inch needle rather than the appropriate 1 inch needle length. Called the patient the day after vaccine to alert him of the error, and he did not report any adverse effects. Offering to repeat vaccine per ACIP recommendations; however, patient has not returned for this.
